Here are your candidates:

For #25039 (Iridium 43), unpredicted Iridium flares are not unheard of - but
not common either. This is in a polar orbit.

The only one in an equatorial orbit (W -> E) was #6153 (Copernicus, COSPAR
72065A).  I've never heard of it flaring but maybe someone else has.
