    I was going over the OIG Catalog Action Report for the week of June 16. The 
    first 22 entries under 'reported as cataloged' are all in sequence, listed 
    from the same source; 1975-052  DELTA 1  US  launch date: 1975/06/12. 
    
    My question is: Is it normal to have so much debris from one source, or does 
    this signify some sort of major failure in orbit, or what???
    
    I'm very curious about this.
