Just a quick thought: If one piece of debris came off the external tank, why not more later in the flight (not seen on video due to camera distance)? When the Shuttle was going much faster, say mach 5 or mach 20? Maybe the air was thick enough and fast enough to accelerate a piece of foam to high enough speeds to damage the carbon-carbon leading edge of the wing.
