where satellites orbit

Some questions,

When a company or a country decides to launch a new satellite into space do
they let everybody else know where its orbit is going to be? How about
American companies? Do they have to apply with NASA for a certain orbital
plane? Are there restricted areas where nobody puts a satellite--like in the
plane of the ISS?

Since so much is being put into orbit I would think that significant
forethought needs to be done about where the satellites are going to be one
compared to another.

Then as they decay the orbit changes. I would think that it is becoming
quite difficult to manage although maybe the density is so low it isn't a
significant threat yet.
